Application process

  • Call; appointments are preferred; it is recommended to call early in the morning if a same-day appointment is needed- Walk in: Weekly STI Testing only: - Fremont Clinic: 1 pm - 4 pm Tuesday, Thursday, and Friday - Central Clinic: 1 pm - 4 pm Wednesday- For health insurance questions, call the billing department. Patient advocates will help those eligible to enroll in Medical Assistance and other public health insurance programs.

Fees

  • Paid by insurance - Individuals with no insurance who verify income will be put on a sliding fee scale according to family size and income. No one is turned away due to inability to pay. - NOTE: Minimum payment is $20

Offers a full range of primary healthcare services for patients of all ages. Primary and Preventive Care:- Adolescent healthcare- Gender care and gender-affirming primary care- HIV care including screening, medication management, lifestyle, and referrals to community resources- Immunizations and flu and COVID-19 vaccines- Minor surgical procedures- Nutrition counseling- Pediatric ADHD evaluation and treatment- Pediatric care/well-child exams- Physical/wellness exams- Prenatal and postpartum care- Screening for diseases such as cancer, tuberculosis, diabetes, and high blood pressure- Sexually transmitted infection (STI) testing and treatment- Treatment for cold/flu symptoms and seasonal allergiesTelehealth services include:- Birth control consultations- Chronic disease management- Cold/flu symptoms- Medication management- Mental health and substance abuse concerns- Rashes and skin conditions- Sore throat/cough- Upset stomach- Urinary tract infections
