Individuals who are residents of Dakota County, have at least 30 days left of their sentence, and have been involved in the criminal justice system.

Application process
- Community Corrections reviews requests from jail inmates seeking re-entry assistance, conducts interviews to determine eligibility and identifies service needs for inmates selected for the program. - After an inmate is accepted, Corrections identifies and coordinates service needs, options and develops a re-entry case plan. - Once the inmate is released from custody, the next step is to assist with his/her transition back into the community, and to follow the inmate for up to 90 days after release.

Helps jail inmates with histories of multiple jail stays minimize the number of times they return to jail. Provides transitional and supportive services that assist sentenced inmates exiting the Dakota County Jail with re-entry into the community.
