- Call for appointment- Patients should present with Insurance, Medicare or Medicaid information and a photo ID of some sort- Patients are asked to complete a health history- Parents should accompany children except where minors are permitted by Minnesota Statute to be able to request care without parental permission- Patients should bring ALL medications in their original bottles- Patients should pay co-pay at time of registration
