- Call for appointment - Patients should present with Insurance, Medicare or Medicaid information and a photo ID of some sort - Patients are asked to complete a health history - Parents should accompany children except where minors are permitted by Minnesota Statute to be able to request care without parental permission - Patients should bring ALL medications in their original bottles - Patients should pay co-pay at time of registration
