Provides older adults who are African American with services that promote healthy aging in their home and community. Services may include: - Economic counseling - Family mediation - Group transportation for necessities (i.e., groceries, prescriptions, household items, etc.) - Health care coordination - Housing stability services - Immigration issues - Mental health support
