Provides older adults who are African American with services that promote healthy aging in their home and community. Services may include:- Economic counseling- Family mediation- Group transportation for necessities (i.e., groceries, prescriptions, household items, etc.)- Health care coordination- Housing stability services- Immigration issues- Mental health support
