  • Adult caregivers or informal providers of in-home and community care for an older adult 60 years of age and older or an individual of any age with Alzheimer's disease or related dementias- Serves African-American and East African residents of Hennepin County

Caregiver support and education services are free; healthcare services are eligible for Medical Assistance and the Alternative Care (AC), Community Access for Disability Inclusion (CADI), and Elderly waivers
