Eligibility

Adults ages 18 years or older with a mental health diagnosis and a demonstrated need for mental health treatment such as multiple hospitalizations or involvement in community programs, or frequent use of crisis services

- Complete and fax intake form located on website - Call for intake appointment; visit website for additional information - Have Medical Assistance. The only commercial insurance accepted is HealthPartners. If you have MinnesotaCare, call (612)314-1100 before making a referral.

24 hour residential treatment. Services include: - Crisis management - Family education - Illness management and recovery - Independent living skills development - Integrated services for chemical dependency - Mental health counseling - Preventive health care education - Recreational and leisure skill training - Resident council - Support groups - Transition and discharge planning
