Eligibility

Homeless adults in need of health care. Serves anyone living in Anoka; Carver; Chisago; Dakota; Hennepin; Isanti; Ramsey; Scott; Washington counties who are homeless or in a transitional living arrangement (i.e., treatment, halfway house, etc.)

8 am - 4 pm Monday - Thursday 8 am - 12 noon Friday

Hennepin County Health Care for the Homeless nurse practitioners operate a clinic offering basic health care services. Clinic staff can make referrals for respite care when necessary to support a client's recovery from an illness or medical condition.
