Open to all who have a hearing loss and need amplification and meet the following: - Denied financing options available at the Miracle-Ear store - Exhausted all resources available for hearing health including, but not limited to insurance, vocational rehab, state or local programs, and other charity sources - Household income at or below 200 percent of the federal poverty guidelines - Possess a family commitment to intervention, rehabilitation, and necessary follow-up services

Application form audiogram and medical clearance form within last six months and signed by a physician proof of household income

8 am - 5 pm Monday - Friday
