Application/intake federal and Minnesota form banking account information copy of last year's tax return daycare expenses and daycare provider tax ID photo ID for all adults on the return mortgage documents if purchased a home in the tax filing year Social Security cards W-2/W-2G-Wage, 1099-MISC/1099-NEC-Earnings, 1099-R, SSA-1099-Pension/Social Security Income, 1099-INT/1099-DIV forms

Provides income tax preparation through Volunteer Income Tax Assistance (VITA) for both federal and state tax returns. Tax preparation offered: - In-person appointments - Self-preparation with assistance via URL link - Virtual appointments NOTE: VITA cannot prepare taxes for businesses, partnerships, rental/multiple properties, farm properties, international returns, and/or inheritances.
