- Residents of Hennepin or Ramsey counties who are low-income, elderly, homebound, or persons of any age who live with a disability, and who are actively on the Community Access for Disability Inclusion (CADI) Waiver, Elderly Waiver or Alternative Care Grant programs. - Community Social Work Program serves families who have children in the White Bear Lake and North Saint Paul/Maplewood/Oakdale School districts.

Case management, including comprehensive service coordination and advocacy for individuals receiving waiver services. The Family Case Management Program provides case management for families in the White Bear Lake or North Saint Paul/Maplewood/Oakdale school districts.
