Program is provided in the individual's homes, within the community, or in specialized housing units depending on the program. Services includes:- 24-hour emergency assistance- Case management support coordination- Community integration and skill development- Health and wellness support- Homemaker services- (Individualized Home Support) IHS without training and with training- Integrated Community Supports (ICS)- Night supervision- Personal support services- Recuperative Care program - Respite care- Support with Activities of Daily Living (ADLs)- Housing transition and stabilization
