Eligibility

Adults must:- Have a serious and persistent mental illness- Have a significant functional impairment, frequent utilization of Emergency Departments, or hospitalizations- Have a health plan such as Medical Assistance that covers ACT services- Live in either Dakota or Ramsey County

Call during business hours for information.A provider referral is needed. Individuals should call the 0067 number if they do not have a case manager, care manager, or social worker.
