Application process

Provides health care in a "health home" approach designed to create an ongoing relationship with a primary care provider (PCP); focus on the whole individual rather than a single symptom; enable a PCP to partner with other specialists, health care services, and community services if needed; include individuals and their families to be involved in decisions; work together to plan care based on the patient's needs.
