- HealthPartners Clinics patients having trouble paying their HealthPartners Clinics medical bills- Individuals must first apply for and use all government program assistance- If an individual was denied government assistance based on income guidelines, they may still qualify for the HealthPartners Clinics Financial Assistance Program

Application process
- Call; email completed applications to [email protected]; mail applications to 21112P PO Box 1309 Minneapolis, MN 55440- Visit website for application/registration form. After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. Individuals need to apply again after three months to see if they qualify for the program.
