Eligibility

Residency is established as soon as the decedent moved to Isanti County unless the decedent was a recipient of Medical Assistance at the time of death. In that case the decedent must have resided in Isanti County for two full calendar months to be eligible.

Required documents

Application/registration form if decedent was a public assistance recipient on the date of death, no proof of residency is required unless there is inconsistent information if the decedent has any assets, they must be verified
