8 am - 4:30 pm Monday - Friday
Application process
- If the dentist is an MDA member: Call the Minnesota Dental Association to file a complaint - If the dentist is not an MDA member: Contact the Minnesota Board of Dentistry, Mary Dee Liesch, at (888) 240-4762 or visit www.dentalboard.state.mn.us to file a complaint
