Application process

Usually referral based, but anyone can call for services. A nurse is sent from the agency within 48 hours to do an in home assessment to determine client's needs. The assessment involves head to toe assessment, vital signs such as blood pressure, medical history review, medication review, discussion of needs, etc. The assessment takes about 2½ hours. Depending on the caller's needs, it will take a few days to a week to begin services.
