Required documents
- Valid photo ID (driver's license, passport, state, military, or student ID) verifying the person's identity and their current local address- Youth less than 14 years old require parent or guardian information and signature. If no ID, provide one of the following as proof of current address: postmarked bill or letter, rental or lease agreement or receipt, property tax statement
