- Accepts intakes from clients, doctors, discharge planners, social services or concerned individuals; email; visit website; call or walk in to arrange an assessment performed in the client's home by a nurse - The assessment takes 45 minutes to an hour. Same-day services can be provided once the assessment is completed.
