Older adults who are residents of Blue Earth, Brown, Faribault, Le Sueur, Nicollet, Martin, Sibley, Waseca, and Watonwan counties
Required documents
Emergency contact services: health insurance cards, medical history, medication list, family contact information, advance health care directive, current address and phone number, and a list of medical providers

Application process
- Call for appointment; email; visit website- A social worker or RN Case Manager will meet with the prospective client and explain services. The initial 1/2 hour of this meeting is at no cost or obligation.- To sign up for services, paperwork includes: a service agreement, a release of information, and a HIPPA receipt- Individuals requesting assistance for 24 hours/7 days a week on-call service, Elder Care Services would collect emergency information as listed above- Case managers will be assigned to each individual as the central point of contact

Case management services include:- 24 hours/7 days a week emergency availability- Advocacy and education- Assessments and consultations- Assisting with health care advanced directives- Coordinating and attending appointments- Crisis intervention- Hospital visits and discharge planning- Identifying and coordinating resources and services- Providing case management reporting- Relocation assistance
