Eligibility

Older adults who are residents of Blue Earth, Brown, Faribault, Le Sueur, Nicollet, Martin, Sibley, Waseca, and Watonwan counties Minnesota

Required documents

Emergency contact services: health insurance cards, medical history, medication list, family contact information, advance health care directive, current address and phone number, and a list of medical providers

Application process

- Call for appointment; write; email; visit website - A social worker or RN Case Manager will meet with the prospective client and explain services. The initial 1/2 hour of this meeting is at no cost or obligation. - To sign up for services, paperwork includes: a service agreement, a release of information, and a HIPPA receipt - Individuals requesting assistance for 24/7 on-call service, Elder Care Services would collect emergency information as listed above - Case managers will be assigned to each individual as the central point of contact

Case Management services include: - Identifying and coordinating resources and services - Coordinating and attending appointments - Hospital visits and discharge planning - Relocation assistance - 24/7 emergency availability - Crisis intervention - Assessments and consultations - Advocacy and education - Assisting with health care advanced directives - Providing case management reporting
