Individuals who are experiencing homelessness or are unhoused and in need of short-term medical care, are residents of Cottage Grove, and are:- Ages 21 and over currently receiving Medical Assistance/Medicaid (MA)- Ages 19 and over currently receiving MinnesotaCare- Needing short-term medical care for less than 60 days
