Children's Mental Health - Targeted Case Management Services - Youable Emotional Health

Eligibility

  • Children ages 3 - 17 who live in Hennepin County- Must have a qualifying mental health diagnosis and significantly impaired home, school, or community functioning that has lasted for a number of months, or the child is at high risk of experiencing impairment in areas of their life

Hours

  • Hours: 8 am - 8 pm Monday - Thursday
8 am - 6 pm Friday- Appointment times and locations are set by the family with the Case Manager

Application process

Referrals can be made by parents or guardians, a mental health professional, or the child's school. Call and ask for the Case Management Coordinator.
