  • Enrolled Leech Lake Band members 55 years of age and older- Enrolled Leech Lake Band member any age, with a disability- Residents of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties

Required documents

Tribal Identification card proof of disability medical form signed by physician application/registration form drivers license picture ID proof of address

Tribal Assistance provides financial assistance, up to a maximum of $300 annually for elders and individuals with a disability on a case-by-case basis.Elder Advocates assist with transportation, food shelf visits, grocery shopping, delivery of commodities, medical appointments, and medication pick-up, completion of forms and paperwork (county, social security, health insurance). Will transport and accompany elders/disabled individuals to appointments pending availability of van and staff.
