Delivers short-term, medically necessary care in your home for people who are homebound due to illness, surgery, or injury. Goal is to help patients recover or manage health conditions in their own home, reducing the need for hospital or clinic visits. Services offered: - Home health aide support (help with daily living tasks, vital sign monitoring) - In-home monitoring / tele-health via remote devices (for chronic disease management like CHF, diabetes, hypertension, wound care) - Medical social services (support with emotional, social, and community-resource needs) - Palliative care for symptom management when needed - Physical, occupational, and speech therapy - Skilled nursing care
