- Assertive Community Treatment (ACT) Program: Individuals ages 18 and older who identify as Southeast Asian who are residents of Anoka, Dakota, Ramsey and Washington counties- Other Programs: Individuals ages 18 years and older and families who are residents of Anoka, Dakota, Ramsey and Washington counties, but will accept referrals from other counties

Provides outpatient mental health, wellness, addiction, substance use disorder, and recovery services for individuals and family members of all ages, that is culturally competent, rooted in community support, and connection. Programs provided:- Adult Rehabilitative Mental Health Services (ARMHS): Support individuals in managing symptoms and reaching personal goals through in-home and community-based care. Services are open to individuals and families to help individuals manage mental health symptoms, build everyday tasks, and work toward goals like employment, housing, or connection to community resources.- Assertive Community Treatment (ACT): Offers multilingual, multicultural support for Southeast Asian adults living with a mental illness. Services are open to individuals and families to help individuals manage their mental health symptoms, help with medications, housing, employment, and recovery.- Targeted Case Management: Coordinated care and community support for adults. Offers one-on-one support for adults living with a mental illness. Case managers connect adults with mental health care, medical services, housing, and community services.- Therapy Services: Personalized trauma-informed therapy for adults and families to help clients navigate life's challenges such as anxiety, depression, trauma, grief, parenting challenges, loss, conflict, and relationship issues. Services are provided in-person and telehealth.- Recovery Services: Strength-based care for individuals and families navigating substance use and recovery. Offers an intensive outpatient program providing same-day evaluations, relapse prevention, and peer services. Group therapy services are offered in relation to recovery, providing resources, education, and support.
