No eligibility requirements; Family Tree accepts all patients for minor illness care services, regardless of insurance status or ability to pay

Required documents

Patients will be asked to provide some basic information like name, date of birth, and insurance status no proof of income, citizenship or residency is required

9 am - 5 pm Monday 9 am - 7 pm Tuesday - Thursday 9 am - 4 pm Friday 10 am - 2 pm Saturday
