Children in households participating in one or more of the following programs: - Supplemental Nutrition Assistance Program (SNAP) - Minnesota Family Investment Program (MFIP) - Food Distribution Program on Indian Reservations (FDPIR) - Medical Assistance (MA) - Head Start Additionally, eligible: - Children who are uninsured - Children in foster care - Children experiencing homelessness - Migrant children - Runaway youth
Need a consent form signed by caregiver
Signed program consent form must be completed by caregiver on a yearly basis
