- Home health care program provides continuity of care to individuals seeking health care services from area physicians - Services are available to persons of all ages - Does not need to have been hospitalized previously to receive this service - Services are prescribed and directed by one's physician Professional services of: - Registered nurses/LPN's - Physical Therapy - Occupational Therapy - Speech Therapy - Medical social services - Home health aides - Homemakers - In-home respite Homecare Programs may allow a patient to stay in his own home and prevent unnecessary nursing home placement or hospitalization
